What are Community Health Workers?

A community health worker (CHW) is a frontline health worker who is from the community and trained to listen to an individual’s health, social, and environmental needs and interests and provide resources and navigation to appropriate programs and services. Community health workers are often bilingual, familiar with the communities they serve, and have a good understanding of local resources.

Who is eligible for this program?


Community members who are enrolled with a Medi-Cal health plan and is not currently enrolled in Enhanced Care Management (ECM).

How do I enroll in this program or refer someone in need of support?

To make a referral for someone else, please complete this form.

What should I expect after making a referral or completing a self-referral?

Following a referral, a community health worker will call the individual being referred to complete a simple social needs screening (5-10 minutes). Together, the community health worker and individual will create a care plan to determine next steps, which may include health education, care navigation, and resource linking. The community health worker will continue to engage with the individual with ongoing check-in calls to ensure maintenance of programs and services.

What does it cost to participate in this program?

Community health worker services are covered by Medi-Cal and will come at no additional cost to the individual.

How long does this program last?

An eligible Medi-Cal member can engage in this program for any period—from 5 minutes to 5 hours—and can work with a community health worker over multiple days or multiple months.
